Practice Pro-Forma
We are excited to announce the rollout of a Patient Engagement Programme aimed at enhancing participation in the bowel cancer screening initiative across North East London. Following the success of our pilot programme, we are eager to expand our efforts and collaborate with your practice to improve early detection and prevention of bowel cancer.
Programme Overview
Objective: To increase bowel cancer screening rates across North east London, particularly among vulnerable patient groups without increasing primary care workload.
Key Components of the Programme
Identification of Eligible Patients: Using Eclipse, eligible patients can be identified and prioritised based on elements such as clinical risk and level of deprivation or vulnerability.
Digital Patient Engagement: Patients will be engaged with via the PEGASUS system within Eclipse. This securely sends SMS messages to selected patients, containing a link to a lander page which will educate them on the importance of bowel cancer screening, and how to participate in the programme. This will offer detailed information on what the screening involves and how patients will receive their test kits.
Secure Questionnaire: Patients will be guided to a short questionnaire to confirm their wish to be involved, provide permission to receive a test kit from the Bowel Cancer Screening Hub where they have not already got a kit, and to note any accessibility issues for performing a test.
Bowel Cancer Screening Hub: Eligible and consented patients, who do not have an existing kit, will be securely notified to the bowel cancer screening hub to arrange a FIT kit to be sent to them.
If your practice would like to take part: Please complete the below pro forma, expressing your interest and permissions for patient engagement.
